Hand Off: Guided Project Handoffs & AI Context Review

A guided project handoff tool that helps people transfer ownership of active work by capturing critical context, identifying missing information with AI, and turning it into a structured handoff for the next person.

Context

Important project knowledge often lives in someone’s head, scattered notes, messages, and informal documentation. When active work changes hands, the next person may receive the tasks and files without the decisions, dependencies, workarounds, ownership details, and other context needed to actually continue the work. Hand Off was designed for knowledge workers, project owners, team leads, and individual contributors transferring active work to a colleague during a role change, leave of absence, project reassignment, team transition, or similar ownership change.

Problem

Traditional project documentation can capture what exists and what needs to happen next, but important handoff context is easy to miss. The person taking over may not know why decisions were made, who actually owns something, which documentation is outdated, what dependencies are unresolved, or what the previous owner simply knew from experience. A useful handoff needs to transfer more than tasks. It needs to transfer enough context for the next person to continue the work confidently.

Approach

  • I structured the handoff around the information someone taking over active work would need: current state, next steps, people and resources, and contextual knowledge.
  • I designed guided prompts to surface information that often stays undocumented, including decisions and rationale, workarounds, outdated documentation, sensitivities, dependencies, and “if you only had five minutes” context.
  • I designed an AI review step that evaluates the captured information from the recipient’s perspective and identifies specific gaps or ambiguities.
  • I kept the user in control of the AI review. The system asks questions rather than inventing missing information, and the user decides whether something needs clarification, is already clear, or is not applicable.
  • I designed the final output to consolidate the captured information and unresolved questions into a structured handoff document.

What I Built

  • A six-step guided workflow: Current State → What’s Next → People & Resources → Things to Know → AI Review → Final Handoff.
  • Structured capture for project status, remaining work, ownership, dependencies, people, resources, and contextual knowledge.
  • Optional prompts that help users capture decisions and rationale, workarounds and quirks, outdated or misleading documentation, helpful background, and the most important information to know quickly.
  • An AI-assisted review that identifies missing or unclear context and generates targeted questions without filling in information for the user.
  • A readiness assessment that shows remaining clarification items while distinguishing them from optional opportunities to strengthen the handoff.
  • A consolidated final handoff that organizes the information for the person taking over and keeps unresolved questions visible.

Outcome / What It Demonstrates

Hand Off turns scattered project knowledge into a structured transfer of context. Instead of giving the next person only tasks and files, it helps preserve the reasoning, ownership, dependencies, workarounds, resources, and unresolved questions they need to continue the work. The AI layer is intentionally used as a reviewer rather than an author of missing information. It surfaces potential gaps and asks targeted questions while leaving the user responsible for deciding what is accurate and relevant.

Lessons

  • Good handoffs depend on transferring context, not just documenting tasks.
  • AI is especially useful as a second-pass reviewer that can identify ambiguity and missing information without replacing human judgment.
  • Guided prompts can surface tacit knowledge that people may not think to document on their own.
  • A useful readiness measure should communicate how complete the transferred context is without implying that AI can guarantee a successful handoff.
